Hello all! My district is working on ensuring all 3rd party websites meet data privacy expectations.  In this process, we are looking to provide teachers a form they can submit to request a website/app be vetted for access.  Does anyone have one of these already created that they may be willing to share? We'd love to be able to compare what others are using to support our work. Thank you in advance!

Over the last several years - through ISTE conversations such as this one, my district has created the below linked form which teachers/staff fill out to request new digital applications that are used with students:

Please feel free to utilize whatever is useful to you from the form!  It was definitely put together learning from many others in the ISTE group.
